Player Verification Waiver & Liability Release

Silver Valley Soccer Club Player Verification Waiver & Liability Release 2025 Season By registering for the Silver Valley Soccer Club (the "Club") and participating in any of its activities, including but not limited to, practices, games, and events, the player and their legal guardian (if the player is under 18) agree to the following terms and conditions. 1. Medical Disclosure The player’s guardian confirms that all relevant medical information, including any conditions or physical limitations that might affect the player's ability to participate in soccer activities, has been disclosed to the Club. This includes, but is not limited to, asthma, allergies, previous injuries, and other conditions that could pose a risk during physical activity. 2. Informed Consent & Assumption of Risk The player and their guardian acknowledge and understand that participation in soccer activities carries inherent risks, including but not limited to, injury (e.g., sprains, fractures, concussions). The player and their guardian voluntarily accept these risks and agree to hold Silver Valley Soccer Club harmless for any injuries or incidents that may occur during participation. 3. Medical Treatment Authorization In case of emergency, the player’s guardian gives consent for Silver Valley Soccer Club to seek medical treatment for the player if necessary. Every effort will be made to contact the guardian as soon as possible. 4. Insurance Coverage The player and their guardian acknowledge that Silver Valley Soccer Club does not provide insurance coverage for players. The player’s guardian confirms that the player has valid health insurance for the duration of their participation in the program. 5. Photographic/Video Consent The player and their guardian consent to the use of the player’s image, video, or likeness in promotional materials, marketing campaigns, and educational content created by Silver Valley Soccer Club. This may include publication on websites, social media platforms, or other media channels. 6. Code of Conduct The player and their guardian agree to abide by the Silver Valley Soccer Club’s Code of Conduct, which includes demonstrating respect and sportsmanship toward coaches, teammates, opponents, and officials. Violations of the Code of Conduct may result in disciplinary action, including suspension or removal from the program. 7. Liability Release The player and their guardian hereby release and discharge Silver Valley Soccer Club, its officers, directors, coaches, staff, and volunteers from any and all liability for any injury, loss, or damage sustained by the player during participation in soccer activities. This release covers all claims, whether caused by the negligence of the Club or otherwise. 8. Acknowledgment & Agreement By submitting the registration form and confirming their agreement, the player and their guardian acknowledge that they have read, understood, and agreed to the terms outlined in this waiver. This waiver is a legally binding document. — Silver Valley Soccer Club – Medical Release and Consent By checking this box, I hereby authorize the staff, coaches, and volunteers of Silver Valley Soccer Club to act in their best judgment in any emergency requiring medical attention for myself or my child during participation in recreational soccer activities. I understand that participation in soccer involves certain inherent risks, and I agree to hold harmless Silver Valley Soccer Club, its officers, agents, and volunteers from any and all liability for injuries, claims, or damages that may arise from participation in club activities, including transportation to and from events. I confirm that I (or my child) am physically able to participate in recreational soccer and that I am responsible for notifying the club of any medical conditions or concerns prior to participation. I acknowledge that I have read and agree to the terms of this medical release.
